Zadaka Health is a private pay practice and does not bill insurance directly. A superbill can be provided after each visit for potential out-of-network reimbursement — many clients also use HSA/FSA accounts to offset costs.
Ketamine medication is prescribed through a licensed compounding pharmacy and billed separately from the $250 session fee. Costs vary by pharmacy and dosage. Leor will provide clear guidance at your evaluation.
Yes. The evaluation is required for all new clients regardless of prior experience. It establishes your current baseline and allows Leor to build a plan that reflects where you are now.
Research suggests an initial series of 4–6 sessions for most conditions. Leor will discuss a recommended plan at your evaluation. Sessions are billed individually — you are never locked into a package.
In most cases, yes. Medical evaluation and ketamine therapy are typically HSA/FSA eligible as qualified medical expenses. We recommend confirming with your plan administrator.
Everything: medical screening, two preparation sessions, the full administration session (4–8 hours), two integration sessions, and ongoing messaging support. Nothing is billed separately within the package. Optional add-on sessions are $150 (integration) or $175 (preparation) per session.
